| Eligibility Requirements | |||||
Participants must be between the ages of 6 and 14 years old (cutoff date is August 31st of the current season) and must be entering 1st Grade through 8th Grade in the Fall 2012 School Year. All participants must have a sports physical or electronic signed waiver (which is signed when you register online) on file prior to engaging in a contact sport. If you are submitting a physical, it must be valid through November 2012 and submitted at sign up (copies will be accepted). |

| 1. Please register ONLINE by Monday, February 6th | |||||
| 2. ANYONE interested in participating - MUST ATTEND a Registration Night as follows (please mark your calendars) | |||||
| Returners: Tuesday, February 7th and Thursday, February 9th | |||||
| New: Thursday, February 16th and Tuesday, February 21st | |||||
| Final Pay Night/Last Minute Registratoins: Tuesday, February 23rd | |||||
| *** All above events held @ Oswego High School - Cafeteria - South Side Entrance Door #13 from 6:00 p.m. to 8:00 p.m. | |||||
| 3. Football players will be weighed to determine what level they will be eligible to play at. | |||||
| 4. Bring the following for each participant: - a copy of the child’s Birth Certificate, - a current picture and - a copy of a utility bill to show proof of residency to keep with your registration. | |||||
| 5. Registration Fees will be collected during this time (Cash, Check or Money Order). | |||||
| 6. If the above information is not met, your child(ren) will forfeit their position. | |||||

| 1 Child $220.00 + $50.00 Family User Fee (non-refundable) = $270.00*** | |||||
| 2 Children $440.00 + $50.00 Family User Fee (non-refundable) - $10.00 Sibling Discount = $480.00*** | |||||
| 3 Children $660.00 + $50.00 Family User Fee (non-refundable) - $20.00 Sibling Discount = $690.00*** | |||||
| 4 Children $880.00 + $50.00 Family User Fee (non-refundable) - $30.00 Sibling Discount = $900.00*** | |||||
| Absolutely no refunds after June 30th, 2012 (for 2012 registrations) | |||||
| *** Families that meet their volunteer obligaions for the season will receive a $50 refund after the conclusion of the season *** | |||||
